ODNR Division of Geological Survey
Sinkhole,
Cavern, or Cave Karst Reporting Form
Provide as much of the requested information as possible. Some of the data are essential (* ) to make the report useful. The more complete and accurate the answers to the questions on the form, the more valuable the report. The three most important points for reporting karst features are: (1) to be as certain as possible that the feature you are reporting is a karst feature (however, when in doubt report the feature); (2) to be as accurate as possible with its location; and (3) if it is a new feature, the approximate date it was noticed. If you know the coordinates of the feature (latitude and longitude from a map or GPS receiver), please provide that information as well. The Geological Survey desires reports on suspected karst features of any size, from one foot to 10 feet or larger in diameter.
Please provide the information below and click the Submit button. Or click here [pdf] to download a printable version of the form and then either fax or mail it to us using the contact information on the left side of the screen.
