Facilitator Handbook (.pdf)
This handbook was created by the national PLT offices and individualized for Ohio. Take a look for suggestions on holding workshops, sample documents, and workshop protocol. (Note: this document is over one hundred pages and therefore we do not recommend printing it).
Workshop Proposal Form (.pdf)
To begin the process of hosting a workshop, just fill out a copy of this form and send it to the PLT-Ohio office.

Use this checklist to ensure that your facilitator workshop plans are moving forward in a timely fashion.
Marketing and Advertising Guide (.pdf)
Promoting and advertising your facilitator workshop is key in providing a successful and well-attended event. This document contains some ideas that may be handy in marketing your event to the target audience.
In Kind Service/Expense Reimbursement Form (.pdf)
This form can be used to log the amount of time, gas, and mileage that you donated to PLT while holding a workshop, along with any reimbursement expenses needed.
Facilitator Coversheet (.pdf)
After your workshop is over please fill out this form and send it to the PLT-Ohio offices along with the participant surveys (link below).
Participant Survey (.pdf)
Copies of these should be sent along with PLT materials required for the workshop, however if you need or would like additional copies they are available here. All workshop participants must fill a survey out at the end of a workshop.

If you wish to give participants certificates at the end of the workshop these samples may be used.

Secondary Module Lending Kits
If you are hosting a workshop with a Secondary Module, you can borrow a kit from the State Coordinator’s office that contains supplies to do one or more activities from the module. To borrow one of these kits you must come to the PLT office on Morse Road in Columbus to pick it up; they cannot be shipped. If you are interested in borrowing one of these kits please email the state coordinator at plt@dnr.state.oh.uswith the following information:
- Your name, email address, and phone number
- Name of the module kit that you would like to borrow
- Date that you can pick up and drop off the kit
- Location of the workshop to be held
The kits are lent out on a first come first served basis, so be sure to request the kit at least two weeks before you will need it.
